In a couple of years, The Binding of Isaac series will be a reference to a similar games like these, just like VVVVVV or Castlevania or The Souls series, that's how good this game is.
The first couple of hours will be challenging because it's hard, and you're still learning the game and its items, but after you invested some times into it, you can easily see why a lot of people invested hundreds of hours because every run is not the same, different items, room layouts, among other things.
Would I recommend this game?, Absolutely. If you're into perma-death, rougelike games, i think this is one of the best, if not the best, game out there.
