At the time of writing this, I've played the first trial about twice and here are my thoughts:
I wish the AI detected you more easily and the horror was less frequent but riskier (like enemies chipping away more health). The frequency of jump scares that aren't as punishing takes away from the horror since being blasted from random insane people + the numerous monstrosities trying to hunt you down gets stale after a while. Don't know how I feel about being able to throw bottles and bricks at enemies to defend yourself since the whole DNA of outlast was to either run or hide. That mechanic makes the enemies less threatening since those 2 items are not that hard to come by. They could also do the more with the music and make unique tracks for when each of the enemies detect you, or you see them for the first time or smth (just a shot in the dark). The new sanity mechanic is great, I do hope that they make it so it increases as you play without needing to be gas-ed by the doctor, like for example, the more close encounters you had with the killers, the more it ramped up slowly since the various stages are the creepiest thing for me, having experienced the max stage more often than the lower stages feels weird. The doppelgangers are also great. I can never trust my friends ever again. Overall, the game's pretty fun and polished. Also, runs really well.

Edit: All I had to do was turn the difficulty up and the self defence mechanic isn't as easy to come by anymore.
