I guess you have to press the left mouse button while approaching the ladder, normally you just climb down automatically.. but I guess not.

Honestly I feel you on the Outlast thing with the bugs, I died twice trying to climb down the ♥♥♥♥♥♥♥ ladder in the sewers after you turn the 2 valves. Instead of climbing down like it usually does, I simply fall down and die from fall damage.. FALL DAMAGE.
