I went into this expecting it to be a somewhat linear, story driven rpg, where the scale is kept small due to it being an indie endeavor, but man was I wrong, and I am happy for it.
This game is huge and has tons of content, despite what the lack of a price tag may suggest.

The map is gigantic, beautifully designed, and has tons of varied areas, and interesting things to check out and you can explore mostly everything at your leisure. (It does have a Fallout style enemy level system, where each area has a certain difficulty to it, so be a bit careful about wandering off the "intended path" although it is all manageable (No instakill Cazadors))
It's loaded with quests that are pretty much all a joy to take part in, with intriguing stories, interesting characters, and sometimes unique mechanics thrown in to mix things up. The side quests are largely all enjoyable too.
Not to mention that pretty much every line of dialogue is voice acted in English, and the quality of said voice acting is very high. The game's sound direction has a very professional feel to it. Not to mention the writing, which I'd say is top notch too.
There's new weapons, armors, spells, potions to try out.
There's no fast travel in a normal sense, but instead you get teleport scrolls that take you to specific points on the map, plus a taxi system.
You don't get a follower at any point that I'm aware of, but instead you can find items that are hidden around the map that boost your max carrying capacity permanently, lessening the need for a pack mule. Not to mention the Resident Evil / RuneScape style bank chests that are scattered all around the towns and cities that all have the same inventory so you can store all your stuff and be able to access it in multiple places.
This, and a few other collectibles with their own bonuses make it so that exploring the map, off the beaten path, in bushes, behind houses, out in the woods, on top of cliffs, under waterfalls and in ruins is extra rewarding.
Plus, the world building and map design helped, as exploring the game was never a boring affair. This also alleviated the lack of fast travel. Just wandering to where I needed to be pretty much always yielded something new to discover, or new collectibles and other items to help me on my way.

The only downsides I can think of would be that the game is a bit prone to crashing every once in a while, but quicksaving every few minutes solves that issue. And there were a few occasions where the scripting didn't quite work, and a door that was supposed to open didn't, or an NPC got stuck or flat out stopped working, but the developer console allowed me to bypass and fix the situation in those instances, so they were only minor annoyances.

I beat this game's main story, plus the vast majority of its side quests in about 140 hours. There's still other routes I could go with in the story, as well as multiple other endings I could go for, but I feel like the ending I got on my first playthrough is the one I want to remember out of this experience, so I will leave it at that.

This was an extremely enjoyable and beautiful experience. Thank you, SureAI, for this gem of a game.
