Why I might not sometimes respond: The main reason is I find "How are you?" to be the most trivial & pointless instant conversation killer in the history of humanity. Often, the person asking the question doesn't really care about the answer and the person answering won't give a real reply because they don't know the person enough to open up and confide what's going on in their lives. Conversations would be a lot better if everyone stopped this useless opening conversation exchange :)
