Lots of practice and a good amount of luck. (most) enemies have predictable patterns you can take advantage of. Watch a few videos of people breaking the game so you can better identify which items are worth keeping and what niche interactions are possible. Isaac is pretty luck based though and you'll still need to unlock the items that are actually good.
