The game ban was almost 9 years ago, for Call Of Duty: Ghosts Extinction, a co-op PVE gamemode similar to Nazi Zombies. I tried to modify my save file to increase my "teeth" count which is used to unlock upgrades for guns and armor. I got banned in 15 minutes and received a "Game Ban".

I do not cheat in PVP games, if you don't believe me you're on copium. Cheers!
