Altered Version error while going Online -
My 100% vanilla install works perfectly. There was no social club update with this update. Verify cache does NOT remove non-vanilla files - only fixes corrupt/missing vanilla files. Uninstalling and reinstalling - does not remove non-vanilla files. Only manual removal.

If you are having the problem - try these instructions. ( STEAM VERSION ONLY)
Restart pc and steam - steam had an update (this also closes stuff in memory)
Exit steam, rightclick steam - run steam as admin.
Rightclick gtav in steam, properties, local files tab, browse local files.
( View file extensions if needed - show hidden/protected files if you hide them still...)
Compare your folder/files to the screenshot - delete anything that is NOT in the screenshot. ( Ignore date, time, type, size - leave commandline.txt IF you use it ) ---
Then - delete the highlighted files.
And then ==> VERIFY CACHE. (rightclick gtav in steam, properties, local files, verify integrity)
http://gtm.steamproxy.vip/sharedfiles/filedetails/?id=578295928
===============================================
Verify cache doesn't always "work" - which we've known awhile- might need to do it multiple times. Sometimes background programs like antivirus/security apps can interfere.
Try excluding gtav - or pause/sleep/disable them to verify. And yes, some game controller apps may want/require .dll's to be put in the dir. Remove if you did - and test. -.-

===============================================
IF YOU ARE 100% SURE THERE IS NO extra files in your gtav folder-
AND - verify cache (or R* version -verify) comes back 100% correct and clean-
That means the other ones on your pc might have issues....
Search your computer for other copies of dinput8.dll-
Compare sizes, dates, versions - perhaps the legitimate copy was corrupted/changed.
(OR put a LEGIT dinput8.dll in your gtav folder as a test (matches window/ver/build/etc)
Possibly even run sfc /scannow from admin command prompt.

If you are getting this around a update/dlc release------
Hopefully you've gotten the new update.
It will appear in my documents / rockstar games / gtav / launcher.log as---
"Version: |1.0.1365.1|1.2.3.8|1.0.1365.1|FSP"
If not - here is a link to the newest appmanifest to help force steam to update gtav-

0xc0000005 ( can also be 0xc0000008 , 0xc0000094 )
Yes, this is long - forced to revise it. There is no TL;DR version.
---Tired of people taking the easy way out and lying about it.

That c0000005 error (in relation to gtav and shared with 8000003/c000094) is typical of:
1- Usb devices (controllers, cams, soundcards, mixers, phones, mics, anything USB)
2- Audio/video apps/services (including msi, razer, riva/rtss, GFexp, fraps, overlays)
3- Mods (any and all - remove them - get it working without vanilla style)
4- Drivers (gpu, audio, network, mobo), OS updates, settings - includes autoupdates.
5- Antivirus/security/malware/adware - combo of updates, apps, perms can give this.
6- Hardware degredation, memory/hdd fails, OC's, bios, etc. Save for last - usually.
7- Miscellaneous stuff. Just as important.

These steps are to rule things out that can/commonly cause this. Don't understand these steps? Can't do it? Lack permissions? Say so! They can be calculated into future attempts to help. Once you know the problem/narrowed it down - you can try upgrading, downgrading, uninstalling, and reinstalling the drivers and software for it. For awhile now- some stuff needs to be uninstalled or reinstalled after a OS/driver/install. Annoying, and NOT only a gtav issue (yay win10 /s).

And now to start -
(NOTE - I do NOT care if anything I suggest disabling below "worked before")
(Eliminate variables. Leave them eliminated. Narrow it down/rule it out)

0 --- Steam version only - VERIFY CACHE GOOD GOD.
Exit steam, rightclick steam, run steam as admin.
Rightclick gtav in steam, properties, local files tab, verify integrity of game cache.
Set gtav .exe's to run as admin? Undo it. Changed windows compatability - undo it.
(some users "may" need .exe's as admin - usually, R* versions of the game - YMMV)
(you do this testing by removing all, then test steam, then steam + launcher, steam + launcher + gtav, steam + launcher + gtav + social club) - if on egs or R*, adjust)

1 --- Unplug any usb device that is NOT a keyboard or mouse. ANY. I don't care how much you need it. Unplug it. DO IT. Test gtav. It works? MIRACLE. Now you can plug them back in 1! at a time and test. Found the one causing issues? You can reinstall/update/verify/research drivers for usb/mobo/problem peripherals.

*"Question! What if my gtav install is on external hdd?" (why?) Yes, leave that plugged in, and continue testing. It will be suspect later if you keep having issues (or test/update usb port, cable, drivers, usb powermanagement/disable it, etc) - lists naturally have a start/end - this info had to go somewhere.

2 --- Open msconfig (Start/run or winkey+R). Go to services tab, checkmark "hide MS services", disable ALL BUT R* related. Next- click the startup tab in msconfig (you may need to click to open taskmanager). Disable EVERYTHING there also. This will stop/disable most everything running in the background. Some WILL fight this/re-add themselves after a restart. Apply/restart pc. Do NOT test gtav yet.

Still got apps starting up? You opened anything except steam? Close it. Open taskmanager. Look in both the processes and details tab(show more details). Any above mentioned "common problem" apps running? Razer/MSI/RIVA/RTSS, etc? There really shouldn't be now - if there are - force close them (and uninstall them). Now try gtav. Works? Wonderful.

Now you can re-enable 1-2 of the services/startup apps you disabled. Apply, restart pc - try again. Find the one causing the problem. And then - you should know what to do.

In some cases, things can also be buried in task scheduler for startup. You can dig through it yourself or use some other tool to see 'all startups' across a OS for a bigger picture of everything.

*Some antivirus/security won't let you disable via msconfig.
Some put themselves back. Ignore them for now.

**For MSI/RTSS/etc - you can test custom hooking/no hooking or straight ignore gtav/etc if you are sure they are the problem/require certain version or some combination of drivers/application are necessary for some other reason/not working for you.

3 --- Gtav mods. Remove them. All of them. You think you removed all them? Never used mods? Forgot about them? Doesn't matter. Go through the motions. There is a link here to both versions of gtav - Steam's and Rockstar's . Click the appropriate one for steps to verify your folder is "clean". The steps in there will also verify your folder/files - and the process "usually" works correctly for steam. Sometimes not. The R* version of verify is far less stable/prone to errors - especially if this is your first install with zero proof the game ever worked.
All in all - a fairly quick process (again - provided it actually works as it should -.-)

*Have this crash and refuse to remove the mods? Say so - so I can ignore. Seriously.
(then look up custom gameconfig.xml, corrupt mods, files, conflicts). It is your best interest to verify a clean gtav works and not ignore modifications you made to files. This is especially relevant around updates. -.-

4 --- Driver-wise - the most common offender is gpu drivers and/or settings. They are 'updated' the 'most' for whatever godawful reason, are usually screwed up with OS updates, and/or have "fixes" where you throw files in the gtav folder (which #3 cleaned). Do not use windows update for these. Google your gpu model, find the appropriate vendor/driver website and download them. Preferably, you use DDU to uninstall them in windows safemode, and then CLEAN reinstall them. You CAN try to use their uninstallers and reinstall - however they are NOT always successful. Nope, don't care if you updated them "last week".

If you are having crashes on a new driver, you can also try rollingback/using an older version. I'm not staying on top of driver versions. You say 'But but but - the newest driver gives me 1 fps more!' - this is me --> not caring.

While reinstalling nvidia drivers - you'd be wise to NOT install geforce experience or nvidia HD audio. They've caused issues before and you are ruling things out. Don't add more junk back - if you truly need these - install them later AFTER the problem has been found/fixed. Restart the pc after the drivers are installed - even if it does not ask. No, do not hibernate/sleep. Shut the pc down for a real restart. Once it restarts-

Open my documents. RENAME rockstar games to rockstar games-OLD. This will remove your settings, and saves - incase either are problematic/corrupted. They'll be safe in the rockstar games-OLD. Deleted instead of renaming? That's on you. This step can be somewhat problematic because of security/permissions. Some users may find that gtav is not recreating the renamed folder/subfolders/files. In this case - you can test by manually creating the "rockstar games" folder, and "gta v" and "social club" inside of it.

*Don't use cloudsaves - be responsible for your data. Added here because 'some' are incapable of going down multiple links. You can disable via social club menu ingame, or by deleting cloudsavedata.dat from my docs / rockstar games / gtav / profile~

*Wiped/reinstalled windows to try to fix this? Make sure windows is FULLY (optionals don't count) updated before you update/install drivers. Win10/other autoupdates caused problems before. Didn't wait? Simply go through the driver reinstall process again.

**100+ instances of this now - remove the directx files: Use this .bat file I wrote from the link below. It will move the files to backup folders and it will open a copy of windows explorer for each of those directories. There SHOULD BE 90 files in each - check. To run it - exit steam, rightclick the .bat, run as admin. You may need to hit 'any key' to continue through the pauses. You can observe the progress and look for errors. If you skip this step - say so. I would highly prefer you did it.
(If you're hesitant to run this - rightclick, edit it, and see exactly what it does) Easy peasy.
.bat - https://drive.google.com/file/d/1rJvafaYjKDOZia5IBMeeIOU5sVZPfzKW/view
(And please, quit dropping random files/versions in your system32.. -.-)

After you run the .bat - install directx again. Preferably you grab it from here -
https://www.microsoft.com/en-us/download/details.aspx?id=35
Rightclick - run as admin. Restart the pc afterwards. Try gtav.
( you can also use steam/steamapps/common/steamworks shared/_commonredist/directx/jun2010/dxsetup ) Though some have stated lately having issues with this one and having better results with the websetup.

***Steam GTAV also has steam/steamapps/common/steamworks shared/vcredist/2012. Run the vcredistx86 and x64 as admin - uninstall both before reinstalling them. Have seen cases where repair didn't work (the systems had other issues). The R* version doesn't have the vcredist/directx distributables in the directory (unless they changed it). Google/download/install.

****Audio, network, bluetooth, wifi, blah blah drivers - are sporadic and you should recognize when you fussed with new drivers/devices. Also important if you redid windows and are using "generics". While reinstalling network drivers - sometimes their extra optimization or bloatware software causes other problems. Can also be targeted by malware/adware/etc. Larger more specific link on profile for social club/network related.

***For audio device issues/conflicts - you can also try disabling ALL other audio/playback/record'n devices in devicemanager. Yes, no sound - rule out the drivers/devices.

5 --- Antivirus/security. Got this far? Are SURE you've done everything? You can test by adding gtav to your antivirus/security realtime/scanning exclusions. There's no guarantee this works - just personally have seen it cause problems before. Of course, the apps are all different - some turn off/stealth while in fullscreen/etc - you'll have to learn (ohnoes!) about your AV/security. Temp sleep, disable, or exclude gtav/social club .exe's. If you can't - or just fed up.. uninstall it - outright remove it from the equation. You can reinstall it later. Yes, windows defender counts.

*I would google adwcleaner from toolslib or malwarebytes - and scan/clean your pc. YES, this will sometimes find apps YOU think are ok, or use - and again - I emphasize - I don't care. There are quite a few apps/things in the past that generated c000005 errors that were immediately fixed after a scan/clean.

6 --- Hardware. True "random" crashing. Power issues, memory gone bad, XMP profile on/off, hard drive generating bad spots - there are many random errors that can pop up with these problems, they can be "consistant", they can manifest as changing errors, BSOD's, errors all over the pc, etc. Again, the thoroughness/reliability of this section depends on you and your past gtav experience. Gtav had no update in 3 months? This section is relevant. Gtav updated 3 hours ago and problems since? I'd "move" this section to the bottom of the post - come back later. Scan your hdd/memory for errors (software used is up for debate). Observe your temps. Tinkering with your OC? Don't care - remove it. Gtav can be pretty finicky with OC's (usually pops a d3d_gfx_init error though) - IS worth testing if you've gotten this far. Possibly default bios settings. *includes amd pbo/autooc.

*Yes, people to point to bios firmware as solution. Possible yes. I never outright suggest if evidence says they have NOT done this post entirely. -.-

7 --- This section is unsorted causes. The "majority" have been personally confirmed at some point. Some things here are not covered above.

Cheating/mod use? Others stated a new char fixed probs. YMMV.

Windows 10 spatial audio can cause issues - turn it off.

*manually set pagefile/virtual memory config. Test setting back to windows/system controlled. Run sysdm.cpl, advanced tab, performance/settings, advanced tab, virtual memory/change. Verify drives are not running out of space for windows pagefile to expand into. You can tinker around with size/location depending on your ram. Don't remove entirely - known to cause issues.

*5+ instances where the error was fixed with sfc /scannow from admin command prompt.

*Open eventvwr.msc. Custom view - admin events. Sort by timestamp. Find the newest gtav crash log. Does it mention EZFRD64.dll ? It does? WOOT! -.-
Open windows/USB_vibration/<somenumbers>/
Rename EZFRD64.dll to EZFRD64.quitcrashing - test gtav. Works? Your controller, drivers, vibration has issues. Reinstall, update, research, etc.

*pop'd via corrupt mfreadwrite.dll - eventvwr crash reports show'd evidence of issues with that file as well as throwing 2-3 errors reports per crash. Usually from dumping incorrect files into windows/system32 or gtav folder. Update files properly/get correct versions. See mfreadwrite and c00007b links on my profile.

*Dec2017 - A couple cases now where a missing Media Feature Pack would pop a c00005 instead of the usual c000135 or c00007b. Relevant to windows N/KN versions - and using the correct MFP for the OS/build. Again - links on my profile.

*Since import/export update - some users with OC's on the gpu/cpu/memory are getting this code / random crashes.

*Some fixed with- winkey+R - msconfig - boot tab - advanced options - MAKE SURE DEBUG IS NOT CHECKED. Apply/restart.

*nvidia hd audio in device manager has caused this problem. (I suspect other software devices can cause this but no firsthand experience with them to verify - virtual audio cable comes to mind). Test by disabling any audio/game controller in device manager.

*A couple have placed blame recently on geforce experience/gtav settings / corrupt gtav files. Untested/proven. Verify cache handled one - uninstall geforce exp handles the other.
ALSO includes section 3 in this link ---- http://gtm.steamproxy.vip/app/271590/discussions/0/451848854986690187/#c451848854986692215

*Dec2016 - Latest update causing problems for users with Astril(can be configured), Lavasoft software (ad-aware/web compantion), hidemyip, etc. Uninstall/disable. Google adwcleaner toolslib - download, scan, clean. Should do anyways as a precautionary step. To quote "I don't have malware" with 1600+ "threats" - 27 being programs. Instantly worked/no crashes after cleaning/restart.

*Dec2017 - Some users throwing c000005 in relation to german/non-english languages. These also used to appear appear as 255(ff). Something to eyeball/check/test.
Sept2018 - more people having server unreachable/job download issues due to language. Quite possible a portion of these crashes *currently* could be related. Try other languages or match languages to steam/os. Change language in steam or languagechanger.exe in gtav folder. R* supposedly aware/working on it.

*March2018 - "rumored" c000005's related to R* ip blocking in the hosts file -.-
Most/all evidence/straight up admitted - points to cheating/tampering with hosts.
Anything related to gtav/R* in c:\windows\system32\drivers\etc\hosts? Remove it.
(can just rename the hosts to hosts-hold incase you can't edit it)
Can't do anything with the hosts? Try going to windows safemode - and try to rename again. Is it a giant 1+mb text file? Just rename/test anyways. -.-

*July+2018 - No firsthand experience on this one - not tested enough. Others have come to me stating it helped - YMMV. Could be similar to mydocs permissions/folder issues.
Open c/users/<usernameinwindows>/appdata(it is hidden/system)/local - there is a rockstar games folder - rename rockstar games-old.

*Jan2019- Now 6 confirmed instance of hard drive firmware being a cause.

*Yes - some Rockstar/servers/connection/etc make everyone have these crashes. When that is the case- everyone is crashing - forums are screaming. There have been variations of this after update/tunables - where it has/hasn't been proven that it was fixable on the user's end. Especially- if R* pushed a "fix" to the update. People routinely panic and start fixing stuff before R* can react - confusing things.

*In really "rare" cases (personally seen 3 from 100's), you can open task scheduler, scheduler library, and disable extra startups there. Rightclick - disable. Restart pc. Test. Just putting it here - extra info.

*** Some have exclaimed fixed after going down yet another link/rabbit hole. I haven't verified anything here - the only things that make sense... are things ALREADY mentioned above (malware/adware/adwcleaner/msconfig/hosts - and restarting modem/router/pc)
YMMV -
http://gtm.steamproxy.vip/app/271590/discussions/0/133259227525755057/#c133259227525787698

--- Done.

** NO ** - this link does NOT cover every cause of this error. New software, drivers, updates, issues pop up all the time. I'm not paid to maintain this with 0 day fixes. Nevermind getting people experiencing these problems to be methodical because they are already got the shakes after 30 minutes of crash. Tried and tried? Following the post to the letter? I'm can use teamviewer to sort out your issue(new/old), and/or doublecheck your work. Don't be a shmuck - help me help you and the community. -.-

To reiterate to the R* version users that bemoan support and wander here -

Yes, this error also occurs on R* versions that simply/absolutely refuse to install properly or the cases where -verify fails to correct half/corrupt/and in some cases - missing files from the game dir. You either reinstall, get a working copy from someone, go pester R*, or compare md5 / eyeball verify the .rpf's (usually) against a CURRENTLY known working copy). (In "most" cases - this is common AFTER gtav has an update, and the new update.rpf / and new dlc files aren't downloaded/updated properly)

** If you only do 3 things on this list and say "this dont work" or "waaa its too long" so you skipped stuff - admit so. These posts are absolutely useless and get you nowhere with me. As said - I can teamviewer and try to help. I've been fixing gtav here for 6+ years. Thousands fixed. As a jaded tech - I'll never believe you unless I see it - I've had far too much time wasted by straight up being lied to.

255(ff)
And - its the 255ff... assuming you did everything above - gtav install/settings should be default. Do you have the eventvwr.crash report as well? Over the last year - that error has been caused by mods (directions above should of cleaned those out), programs running, drivers/updates, etc. It isn't specific. Again, certain errors overlap in terms of what fixes what. More commonly have seen the things below - but it also encompasses the c000005 link on my profile... =\

First I'd open up gtav/_commonredist/vcredist/2012 - uninstall them.
(now in steam/steamapps/common/steamworks shared)
Run both .exe's again and reinstall.

For directx - I would highly suggest you grab this .bat file and run it as admin.
It will move 90 directx files (2 locations - 180 files total) so you can guarantee clean files.
https://drive.google.com/file/d/1rJvafaYjKDOZia5IBMeeIOU5sVZPfzKW/view
After you've moved them with my .bat - install directx from the next link - make sure to rightclick/run as admin. https://www.microsoft.com/en-us/download/details.aspx?id=35
I suggest a pc restart afterwards.

Then DDU safemode your gpu drivers, reinstall them clean.
Possibly downgrade a few versions. Restart computer even if it doesn't ask after driver install.

Open my documents / rockstar games / gtav / rename/delete settings.xml

After the above/restart - if you have any razer, riva, msi, geforce software/services running I'd force stop/close/end them. Anything with overlays, anything to do with audio/video- even turning off steam overlay to test. I wouldn't rule out a clean boot as a test either-
Open msconfig - services tab - hide microsoft services - and disable everything else except R* related..
Then startup tab - same thing - disable it all (or startup in taskmanager on win10).

Also worth checking as the errors cycle/change with updates-
*Some fixed with- winkey+R - msconfig - boot tab - advanced options
MAKE SURE DEBUG IS NOT CHECKED. (NO CHECKMARK).
Apply/restart if you UNchecked it.

And - because in the last week it is causing "new/different" problems...
hit Winkey+R - devmgmt.msc - sound/video/gamecontrollers - disable anything nvidia.
(and quite possibly razer, amd hd audio, or go for broke - all of them as a test (yes it kills sound))

And as confirmed below - controller drivers (and even other drivers)...
can cause problems too -.-

**April2018 - more reports that certain languages can also cause 255(ff). Need more testing if it is actual version differences, or simply fixable via language changer.exe in the root gtav folder.

Version Conversion - Steam/R* ---> Epic
*Steam/retail install to epic games gtav conversion. ***forewarning - rumored to need a little download before instructions work - I put this together (see orig post/bottom) before guides were 'precise' - start install and wait atleast 1% before continuing. I will not test the validity).

------Edit----- and yes, to confirm - I started the gtav install in epic launcher. Then force closed launcher via task manager (I do recall it going to atleast 1%). Copied all .rpf's and folders from steam (or retail) gtav install over to the new install (in my case it was d\:\GTAV\ (AND THEN within that --- .egstore\bps\Install <--- copy to location)). I kept the existing files. Started epic launcher and resumed installation - it quickly rocketed up to 90% installed. Will need to check files after it is done just to be sure - but it appears possible to prevent a large portion of redownload.

*got to 93% and it moved files to correct location and started verifying automatically.
*it finished, didn't appear to complain about files - cleaned up the .egstore/bps/install folder) and is letting me launch now.

*launching prompts the reinstall of r* launcher, social club, directx, and vcredists.

edit - uninstalled egs gtav/launcher- afterwards reverted r* launcher back to play from steam

Original pondering/untested post -
If the epic games launcher only facilitates an account creation/link with R* - and still uses the R* launcher to startup and go - you most likely can use a large part of the steam gtav install - just like you can use it for the steam install to retail conversion. Really depends on how epic handles installation (if it does, as the r* launcher will detect files), and if there is a verify files function to correct any other issues.

General steps for steam to r* here -
https://gtm.steamproxy.vip/app/271590/discussions/0/617335934131510296/#c142260895145976010

Although somewhat changed since new launcher - many have reported simply copying the folders/.rpfs over - and pointing the launcher to that location and having the rest fixed.
YMMV.

Version Conversion - Steam ---> Retail
0
Version Conversion - Retail ---> Steam
Yes, you can use the pc cd/retail/"other" install as a base for steam-
(the bulk of gtav content/rpf files are the same / have been since release)

Obviously--- install it (whatever copy).

Then, copy everything IN your current non-steam gtav folder-
Make a folder in steam/steamapps/common/ called "Grand Theft Auto V".
If you are putting it in a steam library - remember that for later.
Paste everything INTO this new folder.

Next- you need the appmanifest_271590.acf - in the link is my most recent copy - Click Me~
Exit steam. (changes to .acf must be done with steam closed)
Go to steam/steamapps - paste the appmanifest file in.
If you are using a steam library, it goes in steam library / steamapps
If you have multiple libraries it must match the location/drive.

Start steam. You should see gtav listed now - it may/may not say install/play.

-then-

Since the steam version and rockstar version slightly differ-
Enable viewing hidden/system files/extensions in the steam/steamapps/common/gtav folder.
Delete ANYTHING that is NOT a folder OR a .RPF file.
(basically all .asi, .dll, .exe etc - verify cache will correct it)

You should also use this screenshot to compare -
If you got files/folders NOT in the screenshot - delete them - and then...
VERIFY CACHE (rightclick gtav in steam, properties, local files, verify integrity of game)
http://gtm.steamproxy.vip/sharedfiles/filedetails/?id=578295928

Once verify is done/complete -
I would also open up steam/steamapps/common/gtav/installers-
Rightclick socialclubsetup - run as admin.

If the above doesn't work- sometimes you need to "start" the download for gtav first. Let it go until it actually begins downloading (not the space allocation) - then pause. Exit steam.
Copy the appmanifest_271590.acf file again - steam most likely saved/changed it.

About:Blank
Are you opening gtav from a steam shortcut or within steam? Tried creating a new shortcut?
A couple things I would do/check - test gtav after each~
1 - Open windows / system32 / drivers / etc - rename hosts to hosts-junk
2 - Google adwcleaner toolslib - download - scan - clean - restart pc - try again.
3 - Open internet explorer, options, security - disable enhanced internet explorer
4 - Control panel - default programs - check and see what .url is set for.
Set for internet brower/explorer if it is not.
5 - Using any 3rd party antivirus/security apps? (includes windows defender - or more importantly - controlled folder access) Temp sleep/disable / or add to whitelist/exclusions.
I'd probably do a registry clean (backup the changes) too with ccleaner -.-

6 - This can also be caused by virus/malware/adware - some will display ads on startup, some - will not. Cleaning of said issues, doesn't guarantee it will begin working again either if they have corrupted/damage/changed things. (related mostly to the previous 1-5 list).

D3D_GFX_Init - a not fun error -
ERR_GFX_D3D_INIT - yet another unfun error. Some have fixed with DDU/reinstall of drivers - either with updates/rollbacks of those drivers (straight "updates" don't always work, and work less when you layer them ontop of other issues). Others- have fixed by removing the OC or downclocking their gpu/cpu/memory. And some others have cured by removing/uninstalling other various software related to OC/overlays - like MSI, riva/etc. Also personally dealt with 2 that appeared to be temperature related. One changed the fan curve for better cooling, the other turned vsync on instead to cap fps from 110+ to 60. Both ran fine after. Others claimed fixed after changing the ingame DX setting, others - using win7 compatability on the .exe's. deleting/reverifying d3d files from gtav folder, deleting/reinstalling dx files from system32, using window borderless, - blah. Even deleting settings.xml from mydocs/rockstargames/gtav has the potential to fix this. None of this rules out hardware degredation/power issues/etc (though immediately happening after update moves it down the list - probably more reserved for new users without a history of the game functioning properly), mods, corrupt mods, leftover mods, etc.

Small update - literally began experiencing this 2 days ago on my gpu.
(caused by hardware getting old/degrading - now fixed - same gpu, story for another time)

-----1 thing I do that "has fixed" this problem many times in past, good place to start--------
(this does NOT mean it is the only fix or that you stop after doing this one thing -.-)
For directx- reinstalling it over the top of possibly corrupt/incorrect versions don't always work. Use a janky .bat file I wrote from this link --- it will move the files to backup folders for you - make sure to exit steam, rightclick the .bat and run as admin.
(yes, you can rightclick/edit the .bat - to look and see what it does -.-)
https://drive.google.com/file/d/1rJvafaYjKDOZia5IBMeeIOU5sVZPfzKW/view
---------------------
After you've moved them with my .bat - install directx again from the websetup - making sure to rightclick/run as admin. (from steam/steamapps/common/gtav/_commonredist/directx/2010)
(preferably - https://www.microsoft.com/en-us/download/details.aspx?id=35 )
I'd suggest a pc restart - but some users report it working immediately after.
---------------------

With the exception of more permanent steps like reinstalling drivers, and necessary directx/game files - the... the others could be viewed as more temporary workaround/fixes. Requires most testing after other updates, driver updates, os updates, game updates, etc - including any other shenanigans in the future, etc -.-

What does the eventvwr crash report say? Does it point to a specific file? gpu driver? dx file?

1200+ post thread for that error. https://gtm.steamproxy.vip/app/271590/discussions/0/611703709830303956/

Low FPS / stutter / consistant pauses (check sections) - Possible fixes -
There are multiple problems since the patch (mid Dec-ish), and multiple fixes~
You cannot follow some of these exactly- you may need to experiment.
There are 3 sections - for 3 different stutter/lag issues. tl;dr at bottom.
( ! I will attempt to keep this list updated as patch/dlcs hit - no guarantees ! )

SECTION 1 )
***SUPPOSEDLY this was fixed*** Mar2016 - You can also google an older driver to manually install. Those with no issues probably never utilized the "generic" win10 drivers.

For controller problems (currently ***fixed - still worth testing in some instances)
Test this first- turn off steam overlay, try gtav ** If it doesn't work - continue~
Using a controller? Unplug it - try gtav. (plug in after its open)
(any other usb devices besides kb/mouse - unplug them too - yes, all of them)
(Some are saying they simply changed the USB port - YMMV -.-)

SECTION 2 )

*TL;DR? Go to very BOTTOM of this post.

For hdmi conflict - audio driver/software issues-- CONSISTANTLY TIMED STUTTER.
Changed settings to low with no results? Do this. Timed lag/stutter/audio cuts? Do this.
Sometimes also causes the world to load improperly / keeps game lagging behind. Do it.
*Sept2016+ -- also similar symptoms with conflicting softwares (also in instructions below)

Re-wrote this section as 50%+ of those it is given to - failed to do half/any. I'm only listing major offenders - offbrands/software devices/mixers can cause this too. Because of this - do not ignore anything - it is all suspect/part of testing/elimination.

This post assumes you've already DDU safemode / uninstalled/reinstalled your gpu drivers.
(Sometimes, new drivers cause problems and rollback/downgrading are worth testing too)
Preferably custom install without *hd audio drivers /sigh.
Also assumed- you've verified cache in the last 30 minutes.
And aren't using default windows audio drivers/updated to ones your device uses.
Unplug any and all usb devices that are not keyboard or mouse.

- - - - Step 1 - - - -
Always first - open my docs / rockstar games / gtav - delete settings.xml
(rename if you truly can't redo the settings -.- idea is reset perms/gpu/other issues)
(too many new gpu upgrades choking right on this alone - do it)
Lets start narrowing it down and verify you do have the problem/conflict ----
Open start/run - devmgmt.msc - open sound, video, and game controllers.
(This is in system/device manager ---- NOT playback devices from systray speaker)
Rightclick any nvidia in there - disable them.
Rightclick any razer in there - disable them.
Rightclick any AMD hd audio - disable them.
Try gtav. Did it work without issues? Great! -----> Go to step 3.
------- Didn't work?---> Keep going.
Go back to devmanager / disable ALL devices in sound/vid/gamecontrollers. Test gtav again.
(Yes your sound will/might disappear - not the point - run the test)
Did it work this time with everything disabled? Great! -----> Go to step 3.
------- Didn't work?---> Go to step 2.

- - - - Step 2 - - - -
Are you using an inline mixer or something related to your headphones/stereo? Unplug it. Are you using mods / or used them? Check to be sure they are removed entirely also. You should also check even if you think you haven't used mods- siblings, forgot, didn't clean -
(For mods- open gtav folder- delete any that is NOT a folder, and NOT a .RPF file- verify cache) (View file extensions if necessary) Link has pictures/directions for mod removal too -
https://gtm.steamproxy.vip/app/271590/discussions/0/1499000547495596089/#c1499000547495630914
Open task manager, start closing software and services based on brand. Test gtav inbetween each brand. Any razer software. Any nvidia software. Any Logitech software. Then, ANYTHING with overlays (steam overlay too), ANYTHING related to audio in any form (including skype/discord/vent/etc). This also includes services------->

Start/run - services.msc - scroll to nvidia and razer - and stop/disable them all - EXCEPT the nvidia display driver and R* library service. Also check startup tab for related programs/disable. (easiest to reboot pc afterwards for a "clean boot"). Test gtav. Did it work? Great -----> Go to step 4.
Didn't work? -----> Unplug every usb device except keyboard/mouse ----->
First time getting this far? Then go back to step 1 - Go through the instructions again.
Are you here a second time? ----->Go read Notes 1, then back to step 1.
Don't disable R* games library service.

- - - - Step 3 - - - -
You SHOULD still have sound at this point. If you do not - establish sound first.
Enable 1 device - test gtav. No sound? Disable it, try the next.
Do this until you have sound and no lag. Got sound? No lag? Great! ---->
Now, enable ONLY 1 of the leftover disabled devices. Test gtav again. Do this until you know which one(s) is/are causing problems. Now that you have names ---- Great! Go play.
Wait, you say you want to use this software/device that is disabled? Go to step 4.

- - - - Step 4 - - - -
Is nvidia hd audio causing the problem? Leave it disabled if you don't use it.
If you do use it - some have had success uninstalling / reinstalling it.
Also be aware that most automatic gpu driver upgrades/installs will reinstall this.
Use custom install and uncheck it if you intend on not using it or don't want it installed.
Test again with your working sound device and nvidia hd audio.

Is the device razer based? Try uninstalling any and all razer software and reinstall newest version. Some had success doing this / to keep using their razer software. If you don't care - leave it disabled, or- also just uninstall the software. If you know specifically which program/service is the issue from step 2 - you can also stop/disable that again.

Is it some other strange software like Virtual Audio Cable or something else unnamed?
I can only suggest uninstalling / reinstalling - or get used to enabling/disabling it. You can do this in device manager - or read below.

- - - - Ending - - - -
If you want to keep these problem devices installed/able to be used - you can try this.
Re-enable all the problem devices/services (after you've reinstalled them).
Run GTAV. Hit alt-enter for windowed. Rightclick systray speaker - playback devices.
(Some users are also disabling the recording devices for external mixers/soundcards)
Rightclick the window - show disabled/disconnected devices.
Manually disable any devices you aren't currently using. You should be able to hear gtav audio in the background and it should be affected as you test the devices.
Try to narrow it down to your working sound device + 1 extra. You'll need to experiment to see just how many/what works together. This ultimately may/may not work for you - sorry.
*See Note2.

- - - - - NOTES - - - - -
Note1- Depending on the system/drivers - sometimes disabling nvidia and razer devices isn't enough and you need to uninstall them (Feel free to try uninstall/update/reinstall) There may even be more - as logitech has also been problematic (and others).

Note2- Some are having success simply disabling EVERYTHING in playback devices and renabling it all -ymmv (rightclick speaker in systray - playback devices)

- - - - Bonus - - - -
ONLY seen this work in a few rare cases - Go to properties of current playback device - rightclick - properties - advanced tab - change playback to 24bit, 44100hz~

ONLY seen this work in a few rare cases - Check the gtav dir for dsound.dll and the dll's passed around as a map fix for AMD gpus. If there - remove them from gtav dir.

Been suggested, but not seen personally except for causing crashes -
Disable spatial sound / windows sonic for headphones.

- - - - Super Ultra Mega Bonus for Speed Scanners~ - - - -
If you got this far without doing anything because it all sounds stupid and dumb -
Go buy a console - or... Go back to step 1. Yes yes, we shouldn't have to do this- blahblah.

SECTION 3 )
--- Below for less drastic fps drops --- like.. 60 to 40... 30 etc.
--- Especially useful after dlc/patches or GPU upgrades.
--- Worth trying if you consistantly crash/lag/degredation after 5-10-30 minutes of playing.

It goes without saying - restart the pc. Some programs if run/closed before gtav-
can actually leave things in memory or cause problems. Restart - it won't kill you.

**With the new launcher, quite a few have mentioned results by going to settings, and configuring it to hibernate. I haven't seen it be useful firsthand, but there are 14324872394723 configurations of pc/software/settings. Who knows.

Exit game to test this one.
Open my documents / rockstar games / gtav. Delete settings.xml.
(rename if you truly can't redo the settings -.- idea is reset perms/gpu/other issues)
(too many new gpu upgrades choking right on this alone - do it)
(Can also try renaming the rockstar games folder to something (to retain saves))
*Also NOTE - you may need to do this a few times as you test down the list.
Or simply uninstall/disable anything that is "optimizing your settings -
(looking at you GeforceExperience, forced commandline.txt / entries, and even being sure you are in the correct location regarding onedrive/etc)
(if you've manually disabled shadows - you'll need to do it again after the above...)

Unplug any USB devices, except keyboard and mouse. (You could test without those too)
If this does help- usb and motherboard drivers may need update/reinstall.

Obviously temps - everyone checks this.. usually.. >_>

You can/should use DDU (from guru3d) to load safemode and clean uninstall your gpu drivers. Then reinstall/update. Use this also to install older versions if these issues began "after" a OS, driver, or game update. RESTART THE COMPUTER after you install drivers. Sometimes automatic driver updates cause issues. Purge and install clean/fresh - this includes custom profile settings for games. Back them up if you must, but nuke all that back to default settings while you sort the fps loss.
http://www.guru3d.com/files-details/display-driver-uninstaller-download.html

Verify you aren't using a junky onboard gpu. If you are on a laptop - you might be able to choose which gpu you use for which application (or all the time usage).
Also worth testing if you run multiple monitors - unplug/disable all but 1.

Some users have had success disabling xbox/game dvr in win10 - YMMV~
Or for Win10 Creator - enable/disable the game mode stuff..
windowskey+G (might need to go to window mode)
(google it - blah blah)

Also in Win10 - test disabling full screen optimization on the gtav .exe's.
Rightclick/properties/compatability tab - disable fullscreen optimization.
Some have had success, others not - YMMV.
Others have had success matching windows desktop resolution to game resolution.

Others are seeing immediately huge results by turning down particles.
(or ambient occlusion.. seems to vary per driver updates)
Same for grass (duh) and advanced settings (yes, try half, 1/4th's,etc -.-)

Some other users have reported success in enabling or disabling shader cache.
(in gpu control panel~ (gains might vary based on gpu/ram/cache drive speed))

In some cases - yes - leftover mods/files can cause performance issues-
(for steam version- Open gtav folder - enable viewing of file extensions)
(Delete ANYTHING that is not a folder AND is not .RPF file)
(VERIFY CACHE)

For mods on R* / Non-steam version ---- Middle of the post.

Have nvidia gpu or using razer software?
Start/run (winkey+R) - services.msc - scroll down to nvidia / razer.
You "should"/can disable ALL of them if you want to be more thorough in testing.
KEEP the nvidia display driver enabled.
Yes, this "should"/can include outright uninstalling nvidia / razer softwares.
(preference to outright removal - as clean update/reinstalls have proven track record)

Some cases - other programs/services running can give issues. Open msconfig - services tab - hide microsoft services. Disable EVERYTHING except R* related. Open startup tab - or startup in task manager. Same thing - disable it ALL. Restart pc (this step is generally reserved for crashes)
If you see results with this clean boot - slowly enable stuff / restart til you find the problem.

Doublecheck your gpu is set for performance in the gpu driver control panel.
Check in windows powermanagement to see if you are set for peformance as well.
Try opening taskmanager and setting priority of gta5.exe to high.
Laptops can also alter performance based on outlet or battery power.

Install vcredists/directx again- located in steam/steamapps/common/gtav/_commonredist.
(vcredistx86, vcredistx64, and dxsetup.exe)

Use this .bat to remove a bunch of directx files. ---download / run as admin (edit to see what it does) Hit space to continue through it - when complete it should open 2 windows to the folder/files it moved - and there "should" be 90 files in each ---> https://drive.google.com/file/d/1rJvafaYjKDOZia5IBMeeIOU5sVZPfzKW/view )
Reinstall directx with websetup (run it as admin as well) - https://www.microsoft.com/en-us/download/details.aspx?id=35

Using antivirus/security program with realtime scanning/protection? Add gtav folder to exceptions. If you mod - I would not suggest this--- unless you are SAFE about it.
*A couple users experienced stutter/lag - and fixed it by cleaning virus/malware/adware.
Adwcleaner from toolslib is decent in cleaning some. Other reputable software works too.

Only use vsync in 1 location - preferably from the gpu control panel, but feel free to test either. Turn it off ingame.
(while setting it as adaptive - just don't use half, it does what it says unless you are specifically configuring to half your fps...). Others are also having supposed success by turning off vsync everywhere and using a frame limiter to cap their fps instead. 30.. 60.. 59/58..
(nvidia inspector.. dxtory.. msi.. gpu drivers.. so on) - Above 144 can be flaky - untested.
Can also test fullscreen vs borderless here.

Check in your gpu control panel for pre-rendered frames - try setting it at 1.
*Untested/unseen - some have "reported" better results changing physx from auto to gpu.

Check your virtual memory settings in windows. I usually suggest matching to doubling your ram if you have 8gb and under. 16gb seems to work ok with less (from my experience)
Others have had good success letting windows manage the size - on a NON-gtav-hdd.
http://www.mcci.com/mcci-v5/support/howtos4.html
Also be sure you have available drive space for the pagefile size to fluctuate if not hard set.

Disable steam overlay (or any overlays for that matter... ALL of them).
(You can go a step further, and delete gameoverlayrenderer64.dll from steam directory-
Make sure steam is running already - it should/will redownload it on restart -
This fixed an issue in dec2015 where "disabling" the overlay - was NOT enough -
til steam - fixed the problem)

Potential problems/solutions in BIOS / YMMV-
cpu clocking / downclocking / throttling / some turn hyperthreading on/off / etc.
(adding XMP because apparently this is too vague for some people)

Self radio also known to cause issues - quit using it while testing.
(yes, this means remove the files/shortcuts/etc etc)

** Also possible for video emulation/devices to cause issues. Citrix is one example in the past to cause probs. You can uninstall to test, or google/you may find other methods to temp disable, or install without problem components. An example -
https://gtm.steamproxy.vip/discussions/forum/1/1743358239830363122/

NEW- Import/export - some are having success messing around with soft shadows.
(and currently with the snow - turn particles down)

NEW- Import/export update has some users having issues with saving settings, saving certain settings, or settings being reverted. Running as admin, or changing/saving settings while in storymode is working for some -others no.

OLD (but perhaps relevant again with updates) - Yesterday (Dec24,2015), multiple friends had Windows10 autoupdate their gpu and audio drivers. This caused fps issues for them in every game. Ended up rolling back the driver or clean reinstalled them to fix. You CAN set windows10 to not update drivers - it will block all drivers. Preferably - do not use default win10 drivers for gpu/audio if you don't like it willynilly updating them on you.

To disable-System - Advanced system settings - hardware tab - device installation settings.

TL;DR~
Uninstall all razer, msi, riva, geforce experience softwares, nvidia hd audio, amd hd audio, logitech (there may be others that conflict -.-), and all mods - and close discord -.-;; - unplug all usb that are not kb/mouse - test gtav between each. Once you find the problem software, you can attempt update/reinstalls/custom installs.

You can also hit winkey+R - type devmgmt.msc - hit ok.
Open sound/video/game controllers - disable EVERYTHING - try gtav.
If it works as it should - you can enable them 1 at a time to figure out the issue.
Section 2 explains the process more thoroughly.

*Note - this is shortened (duh) - more indepth conflicts/lists are sorted out in section 2.

Yes, I can teamviewer and look at it when I have time if you were unable to solve it. This however is only valid if you did sections 2 and 3 - TL;DR doesn't count.

*It should also be understood that os, game, driver, other apps / updates in the future can also be responsible for causing a variety of problems despite not being explicitly named here. -.-

Out of Game memory -
-Scroll down for sections on embeddedalloc or multialloc-

What is the error in my docs / rockstar games / gtav / launcher.log ? Did you do anything else on the system? Update gpu drivers? Reload OS? You say mods "not installed" - that is not the same as mods were never installed. How did you clean/remove them (if you did have any ever).. ?

Most gtav updates (it has been awhile since one was "downloaded"..) don't just pop gtav out of memory errors. In most cases I've seen- it would be the game update + scripthook not updated + people removing mods so the game worked = not cleaned folders = crash. Verify cache isn't the only step as it won't remove non-vanilla files. Making the game sluggish? Sure, it happens on some systems. But, so can OS updates, gpu drivers, background apps - everything is suspect.

I'd also run msconfig, services, hide ms services - disable the rest.
Same for startup tab (or taskmanager/startup) and disable the rest. Apply/restart/test.
For giggles - I'd also disable SLI just to see if there is any issue with it/current/updated driver.
Also mentioned above- check the virtual memory - do not remove the pagefile entirely.
(possibly make it bigger - or adjust to let windows manage it - just to test)

And then- some other links to check-
Look at the top of your error window for more info --------
2 of them are listed below at the ----------------------------'s

----------- Embeddedalloc error --------------------------------------
http://gtm.steamproxy.vip/app/271590/discussions/0/1368380934255552581/#c1368380934255568137

#ERR_MEM_EMBEDDEDALLOC_ALLOC
#Out of game memory. Please reboot and restart the game.

If you you have hours - what has changed? Game just updated? Redone OS? Mods? Drivers? You've already restarted the pc - right? Or... New user? Mod user? Answers here for everyone. -.-

I'd use DDU ( google is hard [www.guru3d.com] ) preferably IN WINDOWS SAFEMODE to uninstall your gpu drivers - and clean update/reinstall them. (this is NOT using windows to update them - this is you using google to download them directly from the nvidia/amd/intel and installing them (or reinstalling via geforce experience if you use that... stuff - AFTER DDU). Doesn't matter if everything else works or not. Do it. Rule it out. Be methodical. This is the most common fix- especially if you already have playtime / haven't "used" mods.
(yes, some amd users cannot easily ddu - uninstall/reinstall/update gpu drivers)

Have you used any other mods/etc? Remove'm all. Everything. Think you removed them all?
(Have mods and DON'T want to remove? Still - verify the game works vanilla - see bottom)
CHECK AGAIN.
http://gtm.steamproxy.vip/app/271590/discussions/0/133261907148906986/#c133261907149315763

And to be thorough - I'd rename my documents / rockstar games to rockstar games-old.
(this temp removes your save games, settings files, etc etc - again, ruling stuff out)
Then exit steam, rightclick steam, run steam as admin - and try again.
When it asks to use cloudsaves - do NOT - rule those out too.

------Other things that might cause this - far less frequently------

Verified cache? (sometimes installing isn't 100% correct) Preferably after verifying no mods.

Multiple gpus/onboard gpu? Temp disable it - either in device manager / BIOS.
Make sure you are using the correct gpu if on a laptop via gpu control panel.
(or if you have multiple gpus... /facepalm). Doublecheck the plugs in back too incase pc moved, your house maid vacuum bumped it and plugged it back in wherever, you get the idea.

Any additional background audio/video/overlay/recording programs? Stop/disable/test.

Fiddling with virtual memory/pagefile settings works sometimes (possibly outdated - where the error pops up is important in sorting that out) - as long as you keep it in some form. Test system managed - test custom. Double your ram size - tinker. Removing the virtual memory/pagefile might/will cause more problems. MAKE SURE TO RESTART PC AFTER CHANGES. -.-

I'd open msconfig services. Hide ms services. Disable rest. Same for startup apps. Apply and restart too. (not rockstar games library service)

Possibly also - reinstall vcredists and directx from gtav/_commonredist folder too - cover the bases. (now in steam/steamapps/steamworks shared) Edit - and after launcher update, also in prog files / rockstar games / launcher / redistributalbes - or wherever you ultimately installed the launcher to.

**** Regarding the desire to not remove mods. Use the above instructions to move anything NOT vanilla - to a backup folder - and test GTAV as VANILLA ANYWAYS. If it works fine, and the problem can be directly caused by mods - start with checking into using an updated/modified gameconfig.xml or other mods like headadjustor or packlimiter (read your mod pages for troubleshooting tips -.-) to increase certain limits ingame. Again - this section is for IF YOU USE MODS AND DO NOT WANT TO REMOVE THEM / WANT TO CONTINUE USING THEM / etc. If you are NOT modding you DO NOT use this stuff.

Have used this info to personally fix *almost all the instance of teamviewer'n in to see/do this stuff myself. *a couple went away/disappeared after needing to go to dinner/etc - either worked/unfriended/hit by a meteor and never returned - no clue. Regardless... I 'can' teamviewer to do this - but preferably not if you are functionally capable of doing it.
Only so much free time to go around -.-

Trying to load storymode and that is all the details you have available? Any past playtime? Any storymode mod usage? Any mod usage in the past that you attempted to clean up? Already verified cache? Restarted pc? Tried anything else?~

Most immediate regarding that error is Redux - and reverting/fixing the gameconfig.xml to vanilla. (or installing a gameconfig.xml better suited to your mod quantity/pc specs)

If you used mods / removed and want to clean your gtav directory-
Delete anything in the gtav folder THAT IS NOT a .RPF.. AND.. is not a folder. Verify cache.
or - a screenshot for you to compare with - if its not in the screenshot.. it isn't vanilla~
http://gtm.steamproxy.vip/sharedfiles/filedetails/?id=578295928

Already did.. or zero mod usage.. ever? Restart the computer. Possibly DDU/reinstall gpu drivers.
