Door way is completely black and not visible at a distance in Harvest
I am using the medium-low preset.

I notice this on Harvest mainly though if I can recall correctly this also happens in Dustbowl, whenever I walk out of the spawn I have to get to a specific distance in order for the spot to show.

Is there any setting I can change to fix this?

That's a map setting, they're portals to limit the amount of space rendered at once
