You will need:
-8 redstone blocks
-2 lapis blocks
-2 signs
-2 stone pressure plates
-1 piece of redstone

Place a lapis block where you want the telepad.
Place a sign underneath the lapis block and make the fourth line the name of your current location (NOT the destination!)
Place the stone pressure pad on top of the lapis block
Surround the lapis block on all 4 sides with redstone blocks
Right-click the pressure pad with the redstone dust to begin linking it. You should get a message.
Make a nother telepad somewhere and LEFT-click it with redstone dust. You should get a message indicating that they are linked.
