It seems to indicate they would at least get the list to pick from. They don't have to pick you.
Search the inventories of friends of this friend?
I wonder if they have to pick the exact moment they upgrade or if there's a notice you can choose from later. Perhaps they only get one chance to pick and their computer went down.

Sometimes if you're agressive with the alt-tab and you come back, everyone is in a frozen standing pose and still moving. That or they're suddenly swimming no matter what until you reconnect. When that happens you're the only lucky one to see it.

Report him on his (your old) account's profile page. Hopefully you can get it recovered, but there is a higher chance of it getting terminated.

Hackers should primarily be dealt with by each server admin.
If its a VAC secured server then Valve will eat their account alive (Its delayed for some reason).
You can report a person on their steam profile page. Doesn't always mean they get punished, but if different people report there's a better chance.

Also, if you're on a specially made server with extreme modifications its kind of hard to claim hax.

Unfortunately this name change thing is a real problem for admins unless they're also watching steam IDs (the status command) before and after the name change. The imposter would be incredibly obvious. If they're banning by name then I think if there is a duplicate it won't work for the admin (as a safety measure to you).

Well thats a new one... and your friends list isn't "full" by any means.

Unless you meant a group membership invite. Once they say "Ignore" the first time (from anybody) steam won't sent them another request for the same group.

If you just invited them to join a conversation, the same probably applies.

There's a checkbox for starting steam in big picture mode and another one for starting when you log into Linux.

Perhaps the date can help you out.
Look at your entire page view of yoru friends list. should say "Friends since: <date>".
It could be you sent the invite years ago and thats the date he accepted, or he sent the invite years ago and thats when you accepted.
Other than that try asking the guy.
If he's not responding remove him. If he re-ads it was meant to be.
