Ideas for Play and Listen for the Mentally ill
So im terminally ill. I haven’t been able to play games recently because I need someone to speak in my ear, like a podcast etc.

The problem is, if I do that, I can’t focus on the game. It’s one or the other. But that is also the only thing that keeps me from not going crazy

Play a game where you don't really have to concentrate, like minecraft on peaceful difficulty.
What you might want to try is any non-story focused game, where the gameplay loop is the most important. These are generally the games where you can tune out and do other things, too. Which games will hit that spot for you, might differ to those that do it for others.

My suggestions would be things like Powerwash Simulator 1 & 2 (where don't need to focus on anything and can ignore the story); Diablo 3, or similar hack n slash (again, can skip the story and start on a level that is safe, your collected gear making you stronger for each subsequent level); or something like No Man's Sky, where there are a wide range of options for tuning the gameplay loop to your comfort level. These can be played multiplayer, but don't have to be (I play each of them single player).
