There is nothing hooking into the actual game process, the game is not even running. So nothing to trigger any anti cheat software.
There were a few games in the past where the developers actually punished you for using SAM because unlocking certain achievements was linked to unlocking stuff in the game, Payday 2 did that for a long time. But even they no longer do this, so its completely fine to use SAM.
Mind you, a lot of users look down on obvious SAM usage, like when you unlock a bunch of achievements at once they will all have the same timestamp so its easy to spot when someone used SAM instead of doing them legit.
I only use it for bugged achievements that did not trigger even so i fulfilled the conditions, in those cases i have no qualms about using SAM.
Star Wars the Old Republic gives all previously earned achievements (external lancher) if you switch to the Steam version. They all have the same timestamp. I switched back to the external launcher because of this.
Yeah, a bunch of games from third party clients like Ubisoft and such do that.
When they added achievements to most of their old games last year i unlocked hundreds of achievements at once because its linked to your Ubisoft/Ea accounts.
A bit annoying but still a pretty rare exception.
