I had that issue following the other guides using dgVoodoo2 with custom patches to the game. At some point, you end up having to go in to kohan's .exe properties and enable compatibility mode for windows, for me that broke the game and prevented it from launching. I had to go in to my registry settings and find/remove the key that enabled that option, even though I'd turned it off in the .exe and even fresh installed kohan. The link I provided above doesn't need you to touch kohan's .exe and enable compatibility fixes for windows, so for me it launched fine.

Exclude kohan.exe from DEP (deep execution prevention). Then restart computer.
