Just as old hardware can run poorly in new games new hardware can mess up old ones.
Older software wasn't geared to deal with lightening speeds and the overload of
information being fed to the game often causes misreads. - It might fix it and it might
not but it's completely safe way to try.

Back in the days it wasn't an issue on slower PCs, but yes, these days with ports and all that pushwalls move +1 extra tiles more which can cause issues like that. Similarly to original Wolf3D's E6M2.
The issue I mentioned usually happens in the first room, the secret filled with lots of chalices. Either that, or you are pushing some of the walls in wrong order, blocking the way to the extra lives, there's like 3 different secrets where you can block one or more extra lives because of wrong pushwall orders.
Yes. Extra lives do count toward the treasure ratio.
