I had the achievement with 3, but I think it's possible to end the game with just those 2 loops you mentioned. I think I dumbly triggered an extra loop because of a scene that went differently what I usually expected!

You could maybe get there faster when you get 1000 Denarii for the medicine, which I think is possible with the golden bow and getting the three chests with it, as well as the old chests in various places. But I haven't tried that.
Iulia always dies in the first loop so you can't avoid having to loop to give her the medicine regardless if getting it causes the loop.

No you don't have to know about it. On your first starting loop, when you have done all (A LOT) that needs to be done, just steal the vial and run for the exit (or into the room where the shoot was guarding to get some cash).
Also you do not need the golden bow for anything, so you can safely skip that part.
Perhaps it's changed in the latest version (1.3)--the golden rule only triggers if you talk to her twice. If you don't talk to her, you can waltz in, grab the tablet, and leave.
