Heartfruit not spawning

While attempting to obtain the secret shotgun, defeating The Saplord did not at all seem to yield a heartfruit. this was with NewGame+ enabled and playing as 13. Idk what to do. I have beaten Saplord 4 different times and have gotten nothing but a softlock.

Same issue, also 13 and ng+. It worked when switching to Sonja. Playing in Dutch, if that matters.
I also noticed some subtitles not spawning, and the group of npcs the big monster is supposed to kill in chapter Chapter 13 just freezing instead of dying.
