So here's the way it worked for me:
First I went through all the additional dialog options about the building.
-It's gonna be demolished
-There's not much left of this building
-You live in this place
After Urban accused Luca of theft, I went through the "shady deal" dialogues (The same way you'd go for the "Should've told me" achievement). I used three upper dialogue options till there was an option "We are on an important mission..." at the bottom.
Then I went through the dialogues related to the Labs.
-We are trying to save the child subjects
-We work with the children
-You're speaking from experience?
-What happened to you at the labs?
-These children are our last hope
-Who's the "Reaper"
After that, I went through all the topics about The Infrareds. (Don't know if that actually matters or not for this achievement but I did go through all those options)
When the conversation ends and the combat encounter with Urban starts you should get the achievement.

I'm so sorry, but i'm afraid that i should ask your help again.
"Living Saint" Achievement. Nobody dies and i successfully stealth through all encounters, but still didn't get this achievement. What else should i do to get this achievement?
1. Dont kill anyone / Avoid all combat
2. Make sure Luca is with you till the encounter with your father and don't betray him.
3. Avoid the "Red Carpet" ending,
It should unlock once you leave with the children.
First, in the apartment, disagree with the mother and she should lock you up in your apartment ("The rebellion begins" and "Free of home" achievements).
When talking to her in the car say she should stop worrying about you ("This isn't over" achievement).
At the Temple, don't let Luca die, don't kill Urban, and don't destroy Gwen ("Disarmed situation" achievement).
During both Waste Plant encounters don't kill Bolts.
At the Labs, during the first conversation with father, when he offers you a way out say
-Mother seized my apartment
-You can't make it all about work
Go along those lines about the experiments
-But at what cost
-We shouldn't be experimenting on innocent children
-There has to be another way
When there's should be an option
-Well... consider this my resignation
and then
-I've made up my mind
After freeing the kids from the Pod the Bolts should show up. And this is very important for both "Living saint" and "I am the Reaper" achievements. If there's only father showed up, bad luck, you have to retry. Bolts have to be there or the achievements won't work.
Choose:
-Just don't hurt the children
and then
-Protect children
You should get the achievement just before the last cutscene with the Infrareds' guy.
And for the "I am the Reaper" achievement the same things are important but you need to do them in reverse:
In the apartment, make the mother trust you so she won't lock up your apartment ("Walked right out" achievement).
When talking to her in the car lie to her that you just went out to stretch your legs ("Straight-faced" achievement).
At the Temple: let Luca die, kill Urban, and destroy Gwen ("With you all the way", "No more Gwen" and "Show's over" achievements).
During both Waste Plant encounters kill all the Bolts.
At the Labs, dialogues are the same. Good luck!
I tried to get these achievements two time in a row. And in both cases i get a bug on last step. when i must to try hack Dad's computer, and failed it, the node show me, that i have 0 tries, as if i already failed it. And i can't open pods with reflections, cause Wilona says that i must free image. But I can't try to hack it, because i have 0 tries!

Hope this helps : ) It was my "Living saint" run. You should start in the elevator at the Labs.
How did you solve this problem? I tried 4 times in a row, and each time gets this bug
I uninstalled the game and then cleared all the leftovers from the registry and the AppData folder. Only that did the trick. Just starting a new game wasn't enough. ¯\_(ツ)_/¯

Yeah, i still didn't get this achievement. But I see that in your save Luca is alive
