why is this game's performance so weird? since 2021.
running it on a rtx 4060 paired with a r5 5600 with 16 gb of ram on a 500 gb ssd ... using 1920x1080 resolution, while playing with epic settings my card is not on 95-99% but barely hits 50-60% of usage, even when I look at the sky and having over 100 fps, it is CPU bounded? because my CPU doesn't even sweat... while I am play I only hit 60-65 fps with stutters.

Yes, Unreal games tend to hit CPU hard (in a badly designed manner) and that is especially true the more NPCs and players are on screen with you.

It helped my old Ryzen 1600x get rid of the stuttering while moving on the map.
It's the second tip, the first one is not relevant.

I recently fix the inconstant framerate by setting EVERYTHING to MEDIUM, LOW or OFF.
Post Process never over medium, UT has some problem loading textures e shaders strait from disk and is really slow, on my end noticed this game for some reason harder for the PC after running for a while in the map it get stuterings here and there.

The original dev team was small and the game engine is probably older than some of the players in this game. There might be other reasons for performance fluctuations but I suspect those are the main two culprits.

What are you talking about they were using UE4 from the start. First time UE4 was released 10 years ago. It was always running considerately worse than UE3 but UE5 made it even worse, with the current trajectory nobody will be able to run UE6 games.
