If you play some of the carnival games where you're tossing things, you'll notice that your characters nails look weird. This is what's called "clubbed nails" and it only happens when you've got low levels of oxygen in your blood for a very long time, it's typically a sign that your heart, or lungs are about to give out; it's also typically described as "not good."

Does this mean your character has an illness that afflicts one of these organs? Are they about to die? If so, it'd give lots of explanations for the weirdness of this games setting but also raise more questions. Is the island populated with robots so that they can't infect anymore? Is it terminal? Will they eventually be bedridden or will they die on the spot?
