Got all achievements except this one. I thought that maybe it requires certain amount of deaths, but after like 30 intentional deaths I still don't have this achievement. What should I do to get the achievement?

After playing my 1st run and exititng out of the game, All my desktop icons were scattered all over the place, instead of neatly lined up. I have never experienced this before after closing a game on steam. Has anybody else expereinced this happening?
