To be honest I like the achievements the game doesn't tell you about way more than actual achievements. Maybe thats just me though.

I was wondering about this, too. On a whim I decided to try this and got the flower, giving it to the nurse. I'm surprised this doesn't have a secret achievement attached to it.

It's long too late for this answer to be really helpful to the original posters, but I want to mention anyhow for anyone visiting here later (plus I want to promote my new guide a little):

There's LOTS of stuff that's in this game that doesn't have achievements--believe me this is the least of them. I recommend taking a look at the "Events" section of my new guide [gamefaqs.gamespot.com] for a list of all of the things that can happen you might have missed out on if you were just "achievement hunting". (See also this discussion .)

Also, thanks for pointing out that flower; I never noticed it until I read this (and had to update my guide just now, accordingly).

The SOS note can still show with the lights on. Harder to see I guess but it still happens.
