The game just won't start. Everything was fine before. Now i click the shortcut and nothing happens. Who has the same problem?

This is confusing the hell out of me. I'm trying to get the achievements for not getting hit once throughout the game, and stage 4 is being a pain in the arse. I'm not sure exactly when the boss takes damage. I thought it was when the door in the centre opens, but despite continually shooting (with Cotton, I add) I struggled to do damage. I'd burned through all my cards trying to avoid hits before I even got to the second stage. What spots should I target to do reliable damage?...
