Finished the game, and playing in the free mode. Only three achievements left to go, but something is wrong.

I've scanned over 550 stars, and the Super Star Scanner achievement hasn't shown up.
I'm certain that I've met the conditions for "So close, yet so far" as well.

Do these achievements only happen in Normal mode, or is there something else going on?
