Yes and yes. Higher mayhem mode has stronger enemies and Mayhem 1-10 has special modifiers you roll that make content harder (Mayhem 11 drops Mayhem 10 guns but without the extra modifiers of Mayhem 10)
So mayhem 11 is super hard but drops vanilla loot?
And how do I tell what modifiers the gun has on it when the text on the gun just says "mayhem level 10"? Cus I could google the meaning of the red text on legendary items to figure out what they all do, but I found nothing on the gun's mayhem level.
Mayhem 11 is the same as Mayhem 10 but there it doesn't have any of the Mayhem difficulty modifiers applied. So it's technically easier than Mayhem 10. It's supposed to have lower drop rates but I think the devs bugged it so it has same drop chance as Mayhem 10.
Higher mayhem guns just have higher scaling than lower mayhem guns. Starting from Mayhem 6+ there are some new legendaries that can drop but otherwise the guns are just stronger.

There's no need to go to TVHM. If you want to transition to mayhem lvls, the easiest way is go to mayhem 0 first, do the first mission of Ava's mystery podcast's series of missions, farm Sumo at the end of the map until you get a re-volter shield, then switch to mayhem 11, go tip moxxi until she gives you a Crit, enjoy.
