Fast Adaptation, Head Hunter, Fearsome and Overwhelm Mechanics
How these perks work in conjunction with area of effect skills (abbr. AoE) or multiple attacks skills (abbr. 3H) is not obvious (see this discussion). I set up some tests with the help of BB-Edit and code editing to find out about it.

Fast Adaptation
This perk will account for all attacks whether they target a same enemy (3H) or not (AoE). A miss gives you a stack, a hit removes it. But the stack is limited to 1 (+8% hit chance) and it's not possible to go beyond that with a same skill use.

Head Hunter
This perk works the same for 3H or AoE.
Only the first hit will trigger it.
If it's a body hit, then one stack is gained (+15% critical chance) and all subsequent attacks of the skill as well as the next attack of the next skill will benefit from the additional chance.
If it's a head hit, all stacks are removed and all subsequent attacks will have a normal chance to hit head. Hits after the first one for a same skill will never modify the stack number.

Fearsome
This one is interesting because it differentiates between AoE and 3H.
It can trigger on each attack that targets a different enemy (AoE) but only on the first hit if the skill targets a same enemy (3H). In that case, it doesn't matter if the first hit does 15 damage or more, the effect cannot be applied on subsequent attacks.

Overwhelm
Same mechanics as Fearsome (except that it works on a miss).

It actually doesn't.
Edit: a more accurate reason is that it targets a same enemy with the same skill.

Can 3H generate 3 morale checks? Fearsome affects only first hit, but if 2nd and 3rd are 15+ do they count?

Seeing that it doesn't stack with any perks really favorably, the only potential reasons to use 3H would be triple wounds vs squishy targets or triple morale checks.

Absolutely! Each of the 3H attack can still trigger normal morale checks on 15+ HP damage. So it's still interesting against unarmored targets (average regular damage is 14.5 per attack). Especially if you have items that further reduce the target Resolve.

Yeah, something like Huge/Brute/Drunkard duelist with famed 3H flail, crystal skull and wearing Direwolf cloak.
Build includes Crippling strike and Executioner. Actually, while we are at it - if injury is inflicted by 1st or 2nd hit, do subsequent hits benefit from Executioner?

Any enemy with a low enough hp pool that 3head could injure even with Crippling is weak enough that you might as well use a real weapon and just kill them.

The only thing 3head is injuring with any consistency is naked goblins and then naked bandits or naked footman/bill man/arbalest.

If you want to land injuries then use a Mace which can actually injure things before their armor is gone.

Even with a famed 3-Head I would say that Crippling is bad.

I like the Head Hunter stuff here more than anything else. I'm gonna use this on round swing and 3h flail bros. Apparently you can be rolling really insane crit chance on multiple targets with round swing without resetting the HH counter.

The idea of injuring with a first strike of 3h flail and then proc'ing Executioner with subsequent hits is hysterical and worth a shot.

I agree that even at it's best 3H would be a gimmick build. Just figuring out what this best would look like.
