Edit: a more accurate reason is that it targets a same enemy with the same skill.

Seeing that it doesn't stack with any perks really favorably, the only potential reasons to use 3H would be triple wounds vs squishy targets or triple morale checks.
Build includes Crippling strike and Executioner. Actually, while we are at it - if injury is inflicted by 1st or 2nd hit, do subsequent hits benefit from Executioner?
The only thing 3head is injuring with any consistency is naked goblins and then naked bandits or naked footman/bill man/arbalest.
If you want to land injuries then use a Mace which can actually injure things before their armor is gone.
Even with a famed 3-Head I would say that Crippling is bad.
The idea of injuring with a first strike of 3h flail and then proc'ing Executioner with subsequent hits is hysterical and worth a shot.
I agree that even at it's best 3H would be a gimmick build. Just figuring out what this best would look like.
