The spoiler-bug is weird, I will most definitely try to fix it asap! :)
I'll do my best to fix and ease the other stuff as well, especially the bathtub.

This one just happened to me too and the doc was all "Jacob, you hardly painted anything..." I was all "WTF?! where did my masterpiece go?" :(
EDIT: I was totally expecting that regardless of what you painted that the doc would turn it over to you and it'd be like all blood and gore and he'd be all "Jacob... this is very disturbing... how could you paint this in such a relaxed and soothing environment?"

Thanks for reporting it here. I've been trying to figure what is causing this but I'm having hard time duplicating it! Do you remember if you used Steam overlay or tabbed to windows or anything like that after painting? And did you play fullscreen or native mode?

(Well... one I missed to screenshot, cause I pressed E too quickly).
I also encountered 3 random crashes on last chapter... (I assume it was PC overload).
Nevertheless, I did positive review for this game, it was awesome.
