I recently tried the goat demon ring, but I think it doesn't work properly... I saw no infighting so i did some research and saw that the insane demons have a red mist. I was more attentive in further playtrough and saw some monsters with a red mist... but they all attacked me. And there was no wanderer so no confusion possible. Anyone else with this trouble?...

So, it sure does seem like to me you interact with 4 items on Unearthly and sometimes the very gates of hell itself open up to drop several witches, multiple rotten enemies on top of you, a rotten clown or two, multiple rotten Faceless, and *so* many everything else.
