"First" means that doing that activity will play a scene you haven't seen yet - some of those are minor though, so nothing too crazy. The good part are the gems you get from those.

You are right that is says it for things you haven't done before, but they define that by "any conversation/scene you haven't seen before". So you could have several Firsts, especially for working at a job: the first one where the boss introduces you to the job, the next one is you starting to get a hang of it, another might be when it's raining so there is more (or less) people than usual, another might be working with a classmate, and so on. Each of those count as "Firsts" to give you extra stuff.
