Note that interior design is really just a side benefit of having 3D fixtures more than anything else.
That's not how the game works. You don't need any fixtures (exterior or interior) to sell a car in campaign. It's totally optional. You can hide the body to give you easier access to the interior. Look for the buttons on the right side of the screen that allow you to hide various parts of the car. Individual doors cannot be opened.
As MrChips said, it's not supposed to be easy to place the individual components for the interior because the game was never designed with that function that in mind. Most people that go through the effort to do complete interiors take days/weeks to complete it (maybe more).
As for me, I made a decent interior once and decided the time spent vs fun had wasn't worth it. I'd rather spend my time tuning cars to perfection than making them look good. To each their own. Good luck and have fun!
