What are the consequences of dying?

I am a new player and after getting like 3 rares in a row I am now afraid of dying. Do I loose everything the character is carrying? Does a new character takes it place? Does the story changes in some way?

1. Yes, your character loses everything after dying.
2. Yes, there will be a random new character to choose.
3. No. This is essentially a rogue-like game, so the story is not affected by the death of one of your character.
