Bug or i am stupid?
When i add 59 to 4100 with the Adder, the green one, it sometimes results in the correct 4159, but sometimes it results with 4519. Also, when adding 400 to 3497, it sometimes results in the correct 3897, but sometimes in 4257. Bug or i am not understanding something?

After i saved the game it seems this bug has disappeared. But it was not a visual big, the game didnt count numbers other than 4159 for the level, which i have been creating

You may be connecting something wrong, which is the #1 cause of "wrong number" outputs. Or looking at your second example, you may be feeding the wrong numbers somehow, meaning the wrong connections could be coming from a previous machine's output.
