There are 5 endings in total, and yes you need to replay the game in order to get them all

What about field chapter? I didn't play it in my first playthrough. Also tried to replay basement chapter and got same ending

There is one ending in the field chapter, you probably got the 3rd ending witch is the "death ending" you can get in every chapter

No problem, In the last chapter you need to find an exit from the field. you need to stay quiet and don't look at the eyes that pop up, by doing that you will eventually find an exit

To get the 4th ending, after you get the key from the hand in the bloody bathtub, do not go use it on the door to the basement. Instead, go back upstairs to Cody's room. Here you will find a note written in blood. Take the note to the candle in Diane's old room and burn it, this will, from my understanding, purge the entity in the doll. Afterwards you simply leave the house through the front door and you will get Ending 4.
