No idea if this interfered, but I always keep Vsync active in Nvidia Inspector, but always off on game menus.
I think it's because I was using nVidia Dynamic Super Resolution (DSR) to run it. I was trying to upscale it to 1440p and output it on a 1080p monitor. I do it with most old games. If you turn AA off and run it in a higher res with DSR, it is way better quality image than 1080p with AA can get you. I've never had any issue with getting the full 165hz refresh rate with any other games though, even other Unreal 3 games, so it's a bit strange I only experienced this problem with this specific game.
That explains it. You should have led with it in your post. Go on the Windows tab where you choose the frame rate and change it there. It goes to 60hz when you use DSR or DLDSR sometimes. I believed for a long time that activating DSR/DLDSR would necessarily set the FPS to 60, but that's not entirely true and people don't know about it, so they never tell you that.
Yeah, but like i said I've never had the frame rate change when using DSR with any other games, so it kinda slipped my mind as it didn't seem the likely reason. I just find it odd that this specific game is the only one that does this on my machine. There may be others, as I haven't tested it with everything obviously, but I know that the frame rate/refresh rate was unaffected in every other Unreal 3 engine game I've used DSR with.
I know I have to do that, it's already set up. I have no issues running 165hz on any game at all (unless it's an old XP or Vista game that freaks out over 60fps) even when I use the nvidia DSR settings. It's seemingly only with MOH:A that I have any sort of issues with being unable to go over 60hz, and when I run it at 1080P i get the 165hz with no issues. It's not a big deal, with my gpu AA options it looks fine at 1080P, so I just played it that way.
