Thanks for taking the time to post a bug report. It's important to specify whether it happens in multiplayer or not. Provide as much relevant information as possible as that will make the bug much easier to resolve. Also if its a multiplayer bug, the timing of when it happened is important so providing the events leading up to it will likely be helpful. Example: Multiplayer bug.
