How do i close the tunnels the prisoners made?

So for some reason, when my inmates gets caught of having a tool on them. They get put into handcuffs/or not and then they just stand forever in the yard. even tho there are free cells and a holding cell. So they are dying since they don't wanna move and my guards doesn't wanna escort them even if they aren't busy....

Do the number of deaths and escapes affect the overall recidivism of the prison in a separate table "Prison Grading"? Or is the percentage of recidivism of the prison affected only by - punishment, reform, security, health? Unfortunately, I could not find information on the wiki about this table, which is quite strange. After all, this is one of the most important aspects of the prison....
