Nevermind I figured it out, you need to do the fire ending instead of the puzzle ending.

7 years later in 2024 and they still never patched this lol, i had to do the same thing just now, i completed game with puzzle ending and didnt get the achievments to kill the last 3 guys, had to reload save and do fire ending to get the achievments to kill the last 3 guys
