My players entered a combat and proceeded to miscast 16 times in a row. I'm not sure if they just got the unbelievable 1/65,536 odds. Even if that's the case it feels like there should be a failure mechanic to compensate?

I would had them get "Kidnapped" or something, but they were fighting giant mantises and a tentacle monster which was seen eating a corpse, so not much room for remorse there

Try out "Nice Dice" in the Campaign Settings menu. We talked about this in our most recent patch notes, but they balance out the true randomness to eliminate those streaks that feel super bad sometimes.
Our group had Nice Dice enabled, and we had a string of 2 hits to 17 misses. Turned it off, and it went back to something approaching a probability curve, 3 hits to 4 misses.

Yeah. To clarify while we're working on resolving this issue, were you playing an essentially new Starfall campaign with Lv1-2 heroes, and this was the GM Guide provided map?

Yeah! I think for now, turning it off while we figure out why it's happening is best. For the 2 hits to 17 misses, was this also a lower level hero GMG battlemap, or was it more of a customized combat scene?
