This game has problems if V-Sync is not enabled, but it's usually just that some items in the game don't interact properly. This doesn't sound like a V-Sync problem, but you could check your video card drivers and make sure that V-Sync is not disabled, or being replaced with something else (for example, AMD has an "enhanced sync" that replaces the regular V-Sync, and using that can cause problems in this game).

Have Steam verify the game files.
Reboot the computer.
Try different (newer or older) video card drivers.
If none of that helps, you could post some system info (CPU, GPU, OS, etc.). It's an old game, so I doubt that any of that is a problem, but I'm running out of ideas.
