Did you do anything specific to trigger this or it happened randomly?

I think you need to load a previous save. But there is a chance the dragon can spawn if you run around the area or hide from Arantir.

You can also just crouch near the altar and spam use key while aiming at the Skull or somewhere lower to pick it up and then place it again, see speedrun strategies for this.
