'For some reason if you start the game with an xbox controller plugged in, it defaults to offline mode. To make unlocking achivements work again, you have to start the game with your controller unplugged and then plug it in after the game is loaded. Hope this helps!'
It sorta made sense to me, seeing as how I sometimes start the game with my mouse, before actually turning on my 360 controller in the main menu, instead of using the controller to start it through Big Picture Mode. Figured I'd just give it a try and sure enough, the achievements started popping up again as I finished more quests. Not for the ones I already finished, sadly, but I can always try again in NG+. Not sure if it was just a coincidence, though. And I only tried it once. Just thought I'd share!
My hero, finally got Requiem of a Killer to pop after the 4th attempt. Had tried a couple times after a Steam patch where bulk buying trading cards wasn't working, so thought maybe it would be a late pop like some achievements do on console. Was also having Xbox One controller issues half the time loading up the game.
Didn't want to try the HID disabling, Big Picture Mode didn't help, verifying files fixed 1 but then didn't pop the achievement on 3rd success. Finally, plugging in controller after game launched has (knocks on wood) fixed Requiem of a Killer & controller issues. Now just need to complete New Game+
