I have 6 subweapons and I have upgraded them all, but there's still an empty spot in the inventory. I also can't get the arsenal and tinkerer achievements to proc. Is there a hidden seventh subweapon somewhere?

I was clearing up the Elden Pixels catalog of games, got all the achievements for their other titles and was cruising this one. But for some reason the dying achievement isn't unlocking. It’s the last achievement I need but no matter how many spikes I run into, either challenge or normal stages, I can’t seem to trigger it. I tried dying the 50 times and beat the stage to no result. As somebody with fat thumbs (apparently) I really detest that twirl of the scythe you can do by holding up and hitting the action button. I find myself accidentally doing it at inopportune times, and I know sooner or later it's going to cost me dearly. Is there a way to map that function to, say, the R trigger or something instead? Otherwise, this is a really fun and engaging game. Challenging without getting (too) frustrating. Really enjoying it so far......
