There's 2 choices on your 1st playthrough and 3rd choice exclusive to NG+. For your convenience, I'd recommend the top two choices on your 1st playthrough.
When you get to Ch. 2 ~ 3, you can access the DLC episodes from the world map, but the level recommendation lends itself to doing them in Chapter 5, besides Berserker's story in DLC 1.

And as for the DLC, do you mean the fight against Berserker when I try to return to the area?
Or to the Jewels that I need to find?

Kinda, that "True" ending is from that NG+ exclusive 3rd choice.

I mean Miyamoto Musashi Berserker's story in Boss's Resplendent Arena in the Keian Command Championship DLC, she's Lv. 45 even if your Iori is underleveled so you can use her to grind for resources like Mahitotsu Mallets or recovery items.
Not the side quest Rogue Berserker fight at Yoshiwara you should win by the beginning of Chapter 2 (much easier once you unlock Wind Stance).

If I want to play in the DLC when is a good time to try it and not get spoilers?

Like I said, Chapter 5. You will have all your tools to deal with it easily, and you'll meet the requisite levels of 45, 50, and 60 for each DLC.
You can't spoil yourself with DLC 2 or DLC 3 since they're stand alone, while DLC 1 requires story progress to even access some characters, intentionally preventing spoilers.
DLC 1 Default = Berserker, Saber, Archer
Assassin = Ch. 2 Clear
Caster = Ch. 4 Clear
Rider & Lancer = Ch. 5 Clear

When do I need to make the choice?

First playthrough should be Ch. 3 Todoroki + Ch. 4 Fight Tsuchimikado Yasuhiro for Rogue Saber's Digressions in Final Chapter. Third choice is in NG+ Ch. 5.

I want to avoid spoilers so I did not read the guide.
About the choices... I already made some, at some point in the story, is there any effect on the plot or only the last one is impact on the plot?

Ch. 3 choice between Todoroki and Kawasaki Daishi has minimal impact besides the Digression, Ch. 4 & NG+ Ch. 5's choices diverges the Final Chapter plot.
