Am I missing a column?
On the 5x5 ? tab, I have 12 columns: The 11 columns M - E', one empty column and then column #. Am I missing something? Is there more to unlock? And how do I unlock it?. Also, It looks like there is room for a 12th row under H.

There is nothing more. The [#] variant, being a combination of other variants, is a different category and is separated from the rest.
There's also the fact that completing levels below it will count towards [#][+], while completing other combinations will count towards [+].
