But it appears to me that I can only have them on "the ground" and not in my inventory.
So, how do I equip something to a "slot" for "back" or "shoulder" and quickly get to it or do I have to always multi-click the character, then the inventory item from there?

This. Unfortunately there's no quick way to get to an item that is in a container or equipped to your character.
I believe this was left as a way to make the game more realistic. You have to strategize what is worth carrying in your hands vs what you have to stop and dig out. This means if you don't have a weapon in your hand or slung across your back, you don't have time to go looking for it if a monitor lizard attacks, for instance.
