I had some difficulties researching and testing out different things to get all three endings so I decided to compile everything I learned into one post to help out anyone who is confused about what to do. This will also double as a guide to get all achievements since the information applies.

There is a split in the road toward the end of the game where you have the choice of heading left down a path that leads to the phone where you can call the police or you can head right past two barriers toward a dead end. I will refer to this as the "split" in this discussion post.



Endings:

Chickened Out - Some might consider this an ending, other might not, but since it's incredibly easy to do I'll just add it. When you first spawn into the game, turn around, go back to your car, and leave.

Bad Ending - Complete the game before placing all five (5) Jizo statues. If you help the dog, make sure you go left toward the phone to call the police on the split otherwise you will activate the secret ending.

Good Ending - Complete the game after placing all the Jizo statues. Make sure to go left and call the police on the split otherwise you will activate the secret ending.

Secret Ending - Complete the game however you want, the only prerequisite is that you help the dog. Once you have helped the dog, continue on as normal but make sure you take a right at the split and walk past the two barriers standing in the path. Continue down that road until you reach the end of it, at which, you should notice something pretty interesting. Just a hint: run.



Jizo statue locations:

(1) Once you start to make your way through the tunnel, you will come to a clearing where rubble is blocking your path and you must find a key to a bulldozer to clear your path. Once you come to the shrine at the top of the hill, make your way inside to acquire a Jizo statue. The corresponding pedestal is behind the shrine.

(2) Make your way to the cemetery and find a small shack-like building in the farthest corner from which you entered. The next Jizo statue is behind that building and the corresponding pedestal is on the opposite side of the cemetery.

(3) After cutting down the tree with the axe to make a bridge over the river, continue down that path until you hear a dog barking. Dart off the beaten path to where the dog is (it's whining will also help you pin-point it's location once you get closer) and help the dog escape from the barbed wire. Once you have done so, the dog will lead you to, and dig up, the next Jizo statue. The corresponding pedestal is back at the barbed wire where the dog was stuck at.

(4) After placing the third Jizo statue, head back to the path and continue up it. The next pedestal will be in plain sight right before the wall separating the well/house area from the previous region. Next to the pedestal (on the opposite side of the trail) is a tree and in it's branches is the Jizo statue. Use the axe one last time to chop down this tree to acquire this Jizo statue.

(5) The final Jizo statue is buried next to the smallest lamp in the well/house area. Head over to the house and once you go up the stairs, take an immediate right and walk over to the corner of the house. There should be a shovel with a note that details where the statue is. The shortest lamp is buried halfway underground making it stand out from the others and is relatively easy to find after a little walking around. The pedestal is also in the well/house area and quite easy to find so I won't detail where it is since it's really not necessary.



And for the fun of it...

Note locations:

(1) When you first get into the tunnel, keep going straight to find a red note.

(2) Go inside the shrine at the bulldozer area to get the next red note.

(3) The third red note is in the back of the bus (after the truck jumpscare, before the cemetery).

(4) The next red note is sitting on one of the gravestones on the row closest to the road.

(5) This white note is right next to the Jizo the dog helps you find.

(6) This red note is in the house, past the bell, and on the banister.

(7) This white note is in the same house as the previous note, just next to the shovel.

(Extra Note) This note is not necessary to earn the "Avid Reader" achievement but is just simply a small extra that can be found by doing the secret ending and going right down the split until you reach the dead end.
