Sorry if this is obnoxious, but have you hired a cleaner to periodically scrub the work/living areas? If the area gets too dirty, people will turn green, throw up, and die. You can check the cleanliness rating in one of the overlays.

Yes, I have a cleaner, the areas aren't dirty (until someone throws up everywhere)

make sure your washrooms have showers in them, that was the issue I was running into. people being stinky apparently leads to sickness

sounds like you are not handling their working hours too well - try 4 on and 4 off to be safe, but i personally use 6 on and 2 off however this requires some good placement of the ameniities in order to get everything done in 2 hours - but its how i do things and very possible.

4 off seems incredibly excessive, but it doesn't appear to matter how frequently I give it, they just stand around until their needs are down to like 10% then panic though they've been on break the entire time.

I'm having this issue too, I both tried giving them no schedule or tasks to see if they sort themselves out to having a 3 on 3 off schedule and most of the time they just walk around doing nothing. They think about the bathroom then eventually soil themselves, contaminating everything. I have to let the police come in and kill everyone just because it's cheaper to rehire and start the cycle again. It's very annoying idk why they don't use the restroom with all the time

that is very strange. can you describe your problem in detail? do you have a bathroom with showers, toilets, sink and a dryer?

I remember i had the same problem but then i gave them everything a bathroom needs, hand dryer shower toilet etc, them washing n not drying their hands was leading to sickness long ago i guess hasnt been fixed?

the bathroom works like a charm when every device is installed

Ya, sent my cleaner to the restroom several times to clean it, but it was faking to look for work, then the cleaner got sick and puked everywhere.
