It all depends on chance even on higher ranked matches. Accident happen all the time, just a lot less. When you have so many things happening at once.

stupid rein,, still, i think this wouldn't really happen in higher ranked matches.. hmm.. got 6 kills before as D'va xD.. poor junkrat and zen, got killed 2 times xD, mercy res, and as she is doing the res, i drop the D.va bomb.. poor souls never stood a chance
